We use this information to identify you, complete your medical records, process insurance claims, and send prescriptions to the pharmacy.

We share your personal information with the imaging center we send the imaging orders to, your pharmacy and labs. In addition, we may share your information with a referring physician and other specialists. WE DO NOT SHARE YOUR INFORMATION WITH ANY UNAFFILIATED 3RD PARTY. WE WILL NOT SELL YOUR INFOMATION. WE WILL NOT SHARE YOUR INFORMATION FOR MARKETING PURPOSES.

SMS Terms of Service

You are agreeing to receive SMS messages from Sundial Medical Center. This includes SMS messages for appointment scheduling, appointment reminders, post-visit instructions, lab notifications, and billing notifications. Message frequency varies. Message and data rates may apply.

SMS is not shared with third parties for marketing purposes.

  • SMS is Not a Secure Channel: SMS is not a secure communication channel and should not be used for sharing sensitive health information. 
  • No Guarantee of Privacy: SMS messages can be intercepted or accessed by unauthorized parties. 
  • Carrier Fees: Standard message and data rates may apply. The patient is responsible for any carrier charges. 

You may opt-out of SMS messaging by contacting our office or replying “STOP” to a message that you receive from Sundial Medical Center.
